I confirm my child does not suffer from any medical condition other than those detailed above.

In emergency situations, I consent to the county acting in my place if the need arises, for emergency first aid or other medical treatment suggested by a qualified medical practitioner. I understand that in such an emergency all reasonable steps will be taken to first contact me or a named alternative contact.

I acknowledge that the county is not responsible for providing adult supervision for my child, except for formal junior golf coaching, matches or competition.

I agree to my child being transported by county representatives to and from venues when she is representing the county.

I acknowledge that the above information is being held on file and computer as part of the NCLGA Junior Development Plan Tracking System.

I consent to the above information being shared with any PGA coach who may be delivering training on behalf of the NCLGA.
